Welcome to Sephardic Adventure Camp!! Below are attachments for our updated health forms. We request this paperwork be handed in no later than June 6th, in order for us to plan appropriately for a safe summer up at camp. Families sending in forms later than June 6th will be charged a $50 late paperwork fee.

Please note the following forms must be filled out by both you and your health care provider. In the interests of assuring your child’s health, safety, and well being, we ask that the forms be completed as thoroughly and legibly as possible:

Part 1: SAC Camper Health Information – Parent must complete and sign

Part 2: Health History and Medications – Parent must complete

Part 3: Medical Examination and Immunization History – Physician must complete and sign

Part 4: Health Insurance Information – Parent must complete and attach copies of insurance card, front and back

Parent Information: Please read our parent handbook carefully for important information about Sephardic Adventure Camp. Handbook includes: communicating with your children while they are at camp, medication and health information, packing lists, conduct at camp and transportation information. If you are coming from out of town, be sure to fill out the transportation information form. Also, the code of conduct form must be filled out before camp!
